FORMAT Fort Worth | Physician Supervised

Peptide + NAD Therapy

A new layer of your wellness.

A physician-supervised wellness program under the direction of Dr. Em Zakhary, DO. Three protocols. Twelve weeks each.

Targeted signals for the body that wants the next layer.

Peptides are short chains of amino acids. Your body uses them as targeted signals to direct specific functions. Build collagen. Support tissue. Regulate metabolism. Improve recovery.

NAD+ is a coenzyme every cell uses to make energy and repair itself. Levels naturally decline with age.

At FORMAT, Peptide + NAD Therapy is built around three protocols, each twelve weeks long. The right protocol for you depends on what your body is asking for and what we hear in your consultation. Not every body is a fit for every protocol. Some bodies are not a fit for the program at all. That is what the consult is for.

THREE PROTOCOLS. TWELVE WEEKS EACH.

Find the one your body is asking for.

Metabolic Reset.

FOR THE BODY THAT WANTS ITS BASELINE BACK.

A growth-hormone supporting peptide stack paired with metabolic and blood-sugar supplements. Body composition, energy, sleep, lean muscle. The protocol for the body that has been working hard without the response it used to get.

Glow Protocol.

SKIN, HAIR, COLLAGEN.

Pairs the Glow Blend peptide with MOTS-C and a collagen-supportive nutrition plan with Julia, our nutrition counselor. The visible work happens because the invisible work is happening first.

Recovery + NAD.

TISSUE, INFLAMMATION, THE SLOW WEAR OF HIGH OUTPUT.

BPC-157 and TB-500 paired with NAD+ administered in-clinic on a weekly cadence. For the body rebuilding after stress, training, surgery, or seasons of depletion. Our highest-investment recovery support.

HOW IT WORKS

Five steps.

Thirty minutes with our medical team. $100, non-refundable, does not apply toward protocol cost.

1

Book a consultation

Tell us what your body is asking for.

2

Goals, history, what you are eating, how you are sleeping, what hurts, what works. The clinician reviews everything before recommending a path.

We recommend, or we do not.

3

If a protocol is a fit, we walk you through which one and why. If a protocol is not the right move, we say so. We only enroll bodies that are clearly a fit.

Most administration happens in-clinic with our team. Supervised take-home is offered after technique is established. Includes a nutrition session with Julia.

Labs at the right intervals. A check-in at the end of the cycle. The protocol either continues, evolves, or rests.

  • Your first step is booking a Peptide Consultation with our Medical Director, Dr. Em Zakhary, DO. It is a thirty-minute, one-on-one appointment where we review your medical history, discuss your goals, and build a personalized protocol.

  • Peptides are short chains of amino acids, the building blocks of proteins, that act as targeted signaling molecules in the body. They help regulate processes like fat metabolism, muscle growth and recovery, skin health and collagen production, and cellular repair. Think of them as precise messengers that tell your body how to function more optimally.

  • Not quite. Peptides are targeted signaling compounds that direct specific biological actions. Vitamins are essential micronutrients that support overall health. They can complement each other, but they work very differently.

  • Peptides are not effective when taken orally because they are broken down in the digestive system. Instead, they are administered via subcutaneous injections (just under the skin) using very small, fine needles. Simple and well-tolerated.

  • We source exclusively from a licensed compounding pharmacy that adheres to FDA standards. This ensures safety, sterility, and medical-grade quality, unlike "research peptides," which are not regulated for human use.

  • Some peptides are FDA-approved for specific medical indications (for example, certain growth hormone therapies, insulin, and GLP-1 medications). Others are used off-label under medical supervision, which is standard in many areas of medicine.

  • In many cases, yes, but this is determined individually based on your health history and current medications during your consultation.

  • Typically, yes. Labs are often required before or in between cycles. The specifics will be discussed during your consultation and follow-ups.

  • Each program typically includes a 12-week (3-month) active cycle, sometimes followed by an off-cycle period for reassessment.

  • NAD+ is a coenzyme every cell in your body needs to function. It is central to energy production, cellular repair, and how the body handles stress. Levels naturally decline with age. At FORMAT, NAD+ lives inside our Recovery Protocol and is administered in-clinic on a weekly cadence so dose, technique, and compliance stay consistent.

  • The consultation fee is $100. It is non-refundable and does not apply toward the cost of a protocol if you choose to enroll.
